Payment & Office Policies

Cost

The cost of treatment depends on the severity of the patient's problem. You will be able to discuss fees and payment options before treatment begins. We have payment plans to suit different budgets. We also accept assignment from most insurance plans and we will file the necessary papers to the insurance company. We work hard to make orthodontics affordable.

Fee Arrangements

The fee will cover the cost for appliances and for services performed in this office during the active treatment followed by an additional one to two years of retention follow-up. Emergency visits during the normal office hours are also included in the quoted fee.

However, additional charges will be made for appliances lost or broken due to negligence, failed appointments, or late cancellations (less than 24 hours notice). Naturally, the fee does not include any general dentistry, x-rays or extractions needed during or related to treatment performed outside this office.

There are several options available for payment of this treatment fee.

  1. Payment in full: A 5% discount will be given if full payment is made at the first banding appointment with a check or cash. The treatment fee less the 5% deduction will be a one time payment of $___.

  2. Extended Payment Plan: 0% Interest In-Office Financing. This requires a 25% down payment of te total treatment fee followed by equal monthly installments. The initial payment of $___ is due at the first banding appointment. The remaining balance of $___ is to be paid at $___ per month for __ months.

  3. Equal Monthly Payments (No initial balloon payment): On Approved Credit. The total treatment fee is divided into equal monthly installments. Prior to the banding appointment, you will be required to submit a credit application with our office, so that we may run a credit check. If approved, the first payment of $___ is due at the first banding appointment. The remaining balance of $___ is to be paid at $___ per month for ___ months. *There must be a credit/debit card on file with the office for automatic monthly withdrawals. If, at any time the card is declined, the entire balance will be due in full within 30 days.

  4. Third Party Financing: This is an outside credit service that requires pre-qualifying by phone or online. There is no initial payment. They have repayment plans from 24 to 60 months with affordable monthly payments, which include a nominal finance charge. If you are interested, we have brochures that will explain all of the details. *If you prefer, and are qualified, this service may also be used as a partial payment option. (ie. To provide the fees required to start Option #2.)*

  5. Family Discounts: Our office also offers discounted fees ranging from 5% to 25% for additional family members who also choose our office for their orthodontic needs.

Insurance

If you have insurance, we will help you to determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance will be arranged for you to pay (see above). Professional care is provided to you, our patient, and not to an insurance company. Thus, the insurance company is responsible to the patient and the patient is responsible to the doctor. We will help in every way we can in filing your claim and handling insurance questions from our office on your behalf.
